LocumJobsOnline is working with AAS Locum Tenens to find a qualified Family Practice PA in Bronx, New York, 10451!About The Position We are seeking a compassionate and dedicated Physician Assistant or Nurse Practitioner to join our innovative Mobile Care Program. This role focuses on delivering high-quality medical care to elderly patients within cluster care facilities through a fully equipped mobile unit, as well as providing direct in-home care services. You will play a critical role in improving access to healthcare for vulnerable populations, enhancing their quality of life, and providing comprehensive care in a collaborative environment.   Location: Travel to cluster care facilities throughout NYC via Mobile Van.   Key Responsibilities: Patient Care: Provide assessments, diagnoses, and management of acute and chronic medical conditions for elderly patients. Mobile Outreach: Travel to designated cluster care facilities in a team mobile unit to deliver care, ensuring patient needs are met in a timely and efficient manner. In-Home Care: Deliver personalized medical services to patients in their homes, ensuring continuity of care and addressing unique health needs Health Screenings: Conduct routine health screenings, vaccinations, and wellness checks to promote preventive care and early detection of health issues. Care Coordination: Collaborate with the interdisciplinary teams, including primary care physicians, nurses, social workers, and facility staff, to develop and implement individualized care plans for patients. Patient Education: Educate patients and their families on health management, medication adherence, and lifestyle modifications to enhance overall well-being. Emergency Response: Respond to medical emergencies and provide appropriate care or interventions as needed. Community Engagement: Participate in community outreach efforts to raise awareness of the program and promote health services available to the elderly population. Schedule: Full-time 40 hours per week, 8am – 5pm, M-F   Compensation: Physician Assistant salary: $165,000 - $178,000. Nurse Practitioner salary $155,000 - $165,000 Benefits: Annual bonus and full benefits package   Requirements: Licensed to practice as a Physician Assistant or Nurse Practitioner in NY. DEA Certification Board Certification Must have a Medicaid Provider ID and have or willing to obtain a Medicare Provider ID.
