Position Summary : The Integrated Emotional Wellness Clinician is accountable directly to the Lead Integrated Emotional Wellness Clinician. This position is responsible for the supervision of Licensed Graduates and Master level interns. This is a full-time position based out of our Conway Health and Resource Center in SW DC.

Salary - The salary range for this position is $75,000 - $85,000 and the offer amount is determined by the candidate's education, qualifications, and experience. Indeed provides its own estimated salary calculator and is not affiliated with COH's range.

Highlighted Duties and Responsibilities:
  • Support and closely coordinate mental health care with the patient's primary care provider and, when appropriate, other treating mental health providers;
  • Screen and assess patients for mental health and substance abuse disorders. Consults with supervisor on difficult to engage clients.
  • Helps identify and address related problems affecting health of client, such as housing, vocational rehabilitation, and social integration, and coordinates service delivery with other health center staff and community providers.
  • Provides crisis intervention per the clinician's licensing and COH guidelines, to include addressing suicidal/homicidal ideations.
  • Educates patients about common mental health and substance use issues and available treatment options.
  • Monitors patients (in person or virtually) for changes in clinical symptoms and treatment side effects or complications.
  • Provide brief interventions using evidence-based techniques such as Behavioral Activation, Motivational Interviewing, or other interventions appropriate for primary care settings.
  • Provides short-term, problem focused therapy for no more than 5 patients, unless approved by the supervisor and for up to 8 sessions per patient as deemed appropriate. Short-term therapy will focus on goal setting, change, and utilizing brief interventions.
  • Completes a treatment plan for all short-term therapy clients by the 2nd scheduled session. Monitors patient progress through the course of treatment, reevaluating and adapting the treatment plan at least monthly from initial treatment plan date.
  • Collaborates with PCPs and psychiatric nurse practitioner's to support patients who are not improving as expected. This collaboration may include treatment plan changes including new medications or psychosocial treatments, or appropriate referrals for additional services.
  • Maintains and documents all patient encounters, including face to face, telephone, and electronic communications, in the medical record in a timely fashion.
  • Completes other duties as assigned by supervisor.


Requirements


  • Graduate level license or above in social work, counseling, or related field required;
  • LGSW/LICSW, LGPC/LPC, required or comparable license. Any providers who are licensed in Maryland who provide a copy of their license and MD CDS to Talent Management are also eligible for MD licensure reimbursement.
  • A minimum of two years of work experience in a clinical environment preferred.
  • Strong organizational skills.
  • Demonstrated ability to collaborate effectively in a team setting.
  • Experience in screening for common mental health symptoms required. Experience in screening for substance use disorders preferred.
  • Experience with assessment and treatment planning for common mental health and/or substance use disorder required
  • Familiarity with brief, structured intervention techniques required.
  • Experience working with underserved population preferred.
  • Ability to work flexible hours, including evenings and weekends, required.
  • Ability to travel between sites required.
